RRH Physicians Clinic Now Offering Chronic Care Management

The Russell Regional Hospital Physicians Clinic offers Chronic Care Management (CCM) services to assist patients in connecting the pieces of their health plan together.

Chronic Care Management is a critical component of primary care that contributes to better outcomes for participating patients that have two or more chronic conditions. Chronic Care Management provides dedicated healthcare professionals that assist patients in planning for better health and staying on track for good health.

Patients receive a comprehensive plan that will help support disease control and health management goals, including physical, mental, cognitive, psychosocial, functional and environmental factors. Patients also get connected to resources and other educational information that they may not be aware of or know how to utilize. Care plans can also assist caregivers who are furnishing unpaid care for a loved one. CCM also provides support to patients in between care visits by having a regular touch point to educate, encourage and assist in supporting the treatment plan. Improving care coordination between patient and provider also assists patients in improving their health and getting questions and concerns answered, therefore, increasing patient satisfaction, knowledge and compliance with treatment plan practices.

The criteria are clients with two or more chronic conditions that are expected to last 12 months or until death of the patient, when those conditions place a patient as significant risk of death, acute exacerbation/decompensation or functional decline. Examples of chronic conditions include, but are not limited to Alzheimer's disease and related dementias, arthritis, asthma, atrial fibrillation, autism spectrum disorders, cancer, cardiovascular disease, chronic obstructive pulmonary disease, depression, substance use disorders, diabetes, hypertension and infectious diseases such as HIV/AIDS.

Program Manager Michelle Moore, RN, shared, "I am pleased to be part of the establishment of this service for our community. This program is a first step to assisting qualifying clients in matching them with the services they need in both the medical sector and the community sector. Increasing their quality of life while they go through the process of dealing with chronic health issues and making sure they have the communication streams they need to feel more connected to their providers and resource partners is key to my role in helping patients in the CCM program."
